1. If a monster with an effect that is an optional eff or a "You can effect" is started in a chain such as the effect of Lightpulsar Dragon once he has been sent to the grave to special summon a monster. How it would miss timing is if it's not the last thing to occur in a chain. So if the last thing wouldn't be the monster going to the graveyard or obeying the effects condition then the effect will miss timing.

1. When another action is happening at the same time as a  optional trigger effect would be activated. If the "when" is not the last thing to happen in the chain, the effect will not activate. Tributing for a tribute summon, synchro summoning, and cards that destroy and have an additional effect such as Soul Taker (LP are gained at the time the effect would activate) are examples of things that cause missed timing. Only "when...you can" effects can miss timing, never "if...you can".

4. An open gamestate is basically when nothing is going on. No chains are being resolved and no effects are being activated. It is the only time spell speed 1 effects such as normal/continuous spells as well as monster ignition effects can be activated. It is also the only time that things such as normal summons, xyz summons, and synchro summons can be performed.
